Insurance
In order keep our office operation cost low, our office
- does not participate any insurance plans
- does not file the claim
- does not accept payment from insurance company
However, we encourage you to seek insurance reimbursement. We will provide necessary paperwork for you to submit. The paperwork includes a written receipt with date of service, diagnosis code, CPT procedure code and medical necessary letter if needed. If the functional vision exam and vision therapy program are covered by your plan, it falls under major medical, NOT vision insurance.
- For functional vision evaluation, there are four possible codes we use for our evaluation: 92002, 99213, 96112 or 92060. Most insurance companies cover it. We still encourage you to check with your insurance company ahead of time to see if your plan will cover these codes.
- For vision therapy, there are possible five codes we use for therapy 99212, 92065,97110, 97112 or 97530 along with diagnosis code. Since the coverage of vision therapy is varied from plan to plan, please check your insurance company before starting therapy. We can provide you with monthly statement which reflect dates of service and cost for the month.
Flexible Spending Accounts or Health Saving Account
Many employers are offering Flexible Spending Account (FSA) options to employees. Vision Therapy generally qualifies as a medically necessary service for FSAs or Health Spending Accounts (HSAs). These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as vision therapy. Check with the benefits administrator at your work to see if you are eligible for this program. FSAs or HSAs can typically be used to offset out-of-pocket costs on your behalf.
Payment options
Payment is due at the time of service. We accept cash, checks, major credit cards and Care Credit. In order to make the vision therapy available and affordable for most families, our office offers several payment options and financial options.
