It depends on your Health plans. If it is covered, it will fall under your medical plan, NOT your vision plan, as all functional vision problems are considered medical issues. Vision care from AVL is a medical necessity.  a  medical necessity letter is available to you upon request.

 Our office does not participate in any insurance plans. We will provide this service as out of network provider and provide a superbill for your to claim your benefit. There is no third party involved to provide this unique service.

For our functional vision evaluations appointment, we may use four CPT codes: 99215, 92060 and 92083. 

For any follow up appointments, we may use those CPT codes: 99213, 99214, 99215, 92083, 92082 or 92065, along with the appropriate diagnosis codes
